You have two Preferred Provider organization (PPO) choices offered through Health Plans, Inc. – A Harvard Pilgrim Company:  the HealthAlliance Value Plan and the HealthAlliance Premier Plan.  You benefit from the greatest discounts and most reduced copays for services received at HealthAlliance and all affiliated providers in the HPHC network: UMass Memorial (all campuses), Clinton Hospital and Marlborough Hospital.  Physical therapy and occupational therapy copays are waived if services are received at HealthAlliance. 
This is included as a component of our medical plan and is administered by CVS Caremark.  The plan offers a discount on copays for 90-day supplies of maintenance medications when filled at a CVS Pharmacy or via CVS Mail Service.  Members enrolled in the CVS Caremark plan receive an ExtraCare Health Card which offers a 20% discount on CVS brand health-related items.
The Delta Dental Value Plan and the Delta Dental Premier Plan provide coverage for a wide range of dental services.  The coverage levels vary between the two plans.  Major restorative services and orthodontia are not covered under the Value Plan (plan year benefit maximum - $1,000).  These services are covered under the Premier Plan (plan year benefit maximum – $1,500). 
The EyeMed Vision Plan offers benefits for eye exams and lenses every 12 months and for frames every 24 months.
As a subscriber in one of our medical plan options, you and your family are eligible for a reimbursement of up to $150 per calendar year per household in qualified health club membership fees.
